How to download:

1. Make sure your are on a desktop or laptop that supports the downloading of Fillable PDF’s (most cell phones do not)
2. Click ADD TO CART
3. Click Proceed to CHECKOUT
4. Enter your information (credit card is not needed as this is a free downloadable item)
5.  MAKE SURE TO CLICK CREATE ACCOUNT if you are on a cellphone so you can retrieve the download later.
6. Click “I agree to terms and conditions”
7. Click Place Order
8. You will then see a screen confirming your “order” click the grey “Download box” on a pdf supported device.
9. If you accidentally close before filling and printing, you can find the download under My account. Log in using your email and password and check downloads.
10. Fill out your form with your custom information and click Print. You may need to select “fit to page” on your printer depending on your printer settings.
